- 3. interventions excluding resuscitation. ICU admission can be considered if best location of delivery of short term symptom directed care.
- 6. mechanism by which health care professionals describe and communicate the general focus of care for the patient.

- 11. an individual who is a member of the regulated health discipline
- 13. provided if GOC designation is not documented on the patients health record
- 14. process by which HCP or alternate decision makers consider clinically indicated future care for patient.
- 15. Only R1 & R2s may receive this intervention

- 1. any member of the health care team may initiate or document this.
- 2. considered when necessary, GOC may be revisited as a result
