- 2. I am a cheerleader, and I attend practice/events anywhere from 3-5 days a ____ for 2.5-3 hours at a time. Outside of practice time, I also work out independently 4-6 days a week, usually for 15-20 minutes.
- 4. I try to go to bed before 11 to ensure that I get enough sleep, but if I can’t fall asleep before 11, I at least make sure that I’m lying down so that my body can still ____.
- 8. I often forget to floss my teeth for months at a time, so I’ve been trying to start flossing once a week, and once I’ve reliably built that habit I will increase the amount of times I _____ per week.
- 10. All things considered, I am very mentally stable. I used to have to take ________________ but I haven’t had to take them for quite a while now. I also used to skip school frequently because of my depression (mostly in middle school, but I also skipped a few times last year), and I was able to gradually turn ‘missing the whole day’ into ‘missing half of the day’, which turned into ‘missing first period’ which turned into ‘arriving five minutes late’ which turned into ‘arriving five minutes early’. Of course, this isn’t a habit that I’ve broken completely; there will probably be a few days this school year where I have to arrive late or miss a few classes. But for the most part, I am able to get out of bed and attend school on time! :)
- 11. When someone says or does something that upsets me, I really struggle with telling them. However, _____________ is very important for maintaining good relationships and solid mental health. Going forward, I’m going to try to be more confident with my boundaries and let people know if I’m upset with them.

On Maslow’s Hierarchy of _____, I would say I’m at a level 5 (self-actualization) because I’m not really worried about what other people think of me, I’m just trying to improve my health for myself. While I’m not perfect, I feel like I have all of my basic needs met and a pretty good self-esteem. There are always improvements that I can make but I would definitely consider myself to be a healthy individual without significant problems.
- 13. I’m going to balance my mental, social and physical health by trying to improve one thing at a time. It’s really overwhelming to try to make huge ____________ in several areas of your life, so I’m making gradual changes in just a few different areas until those changes become second nature.

- 1. It’s important to reflect on your overall ______ because sometimes your health will deteriorate without you even noticing. Stepping back to say, ‘have I eaten recently? Have I gotten enough sleep? Why am I so stressed out?’ can dramatically improve how you feel in the moment. It can also save you if you notice that you’re not taking care of yourself before you reach a crisis zone.

My diet isn’t very healthy because I don’t eat enough fruits and _________ and I basically eat the same meals every day, but recently I’ve been trying out new recipes I find online to try and include more variety in my diet. I’ve also started trying to eat more nuts & fruits with my meals to ensure I’m getting enough nutrients.
- 5. I have a small group of really good friends that I talk to multiple times a day. I would trust them with information I’d never tell anyone else, and have told them about feelings & experiences that I would be uncomfortable talking to anyone else about. I also have a lot of friends that I don’t talk to quite as often but that I still see/interact with regularly that always bring a _____ to my face.
- 6. I will probably need someone to hold me accountable & ensure that I’m not building bad habits. I’ve been talking to my dad about getting a gym membership and going together so that I don’t take too many days off. My mom encourages me to eat ______ & vegetables when she cooks for me. I will also need to confide in my friends more instead of keeping my problems to myself.
- 7. Overall, I’m not incredibly proud of my design for this ____________. While I feel like it’s original and interactive, it’s not very visually appealing/interesting. Next time, I’ll choose a format that is more fun to look at.
- 9. My best friends don’t go to my school, which means I’m basically completely alone on even days. I think I’m an extrovert, so going the whole day without talking to my friends is a drag. I also don’t have many classes or lunches with my ______ friends, which makes me wish I had more friends at this school. I struggle with making friends because I’m shy but I’m trying to be more outgoing and ‘risky’ with my social life (for example, trying to sit next to someone I kind of know instead of just sitting alone in the corner).
