  1. 2. Frank D Lanterman Regional Center – often referred to as “Lanterman”

Project Homes – referred to as such by RC for homes which provide on-site 24/7 support staff, generally provided by the same agency which has an office in the specific location with 24/7 staff availability to residents. These locations do not receive H+S from HOPE. Daisy, Banner, Redondo and Clark. Also receive RC subsidies for their rent. (Lemon residents also receive rent subsidies from the RC but there is no SLS on site)
  4. 8. Unique Client Identifier – number assigned to each client by RC. This is a statewide number, if a person moves from one RC to another, they keep their UCI.

  6. 11. Individual Program Plan – developed annually or triennially by SC for each client.

  8. 14. Vendor Status Notification – issued to a vendor such a HOPE by the RC indicating what services are allowable by HOPE under that specific Vendor number.

  5. 7. Supported Living Services – Supports paid by RC for residents to develop or maintain skills needed to live independently.
  6. 9. Individual Housing Services Plan – HOPE document which delineates factors affecting housing for Independent Living residents.
  7. 11. Independent Living. Usually residents living in HOPE affordables and most HUD homes. Receive SLS supports but not 24/7.
  8. 12. Purchase of Services, referring to the AUTHORIZATION for services from RC. May refer to it as POS or Authorization indiscriminately.
